VACCINATION REQUIREMENTS FOR ALL UPCOMING 7th GRADERS

Georgia State Law requires all 7th Graders must have a Meningitis (MCV4) and a Tetanus (Tdap) Vaccine on a GA3231 Immunization Record before entering the 7th Grade year including Forsyth Virtual Academy students.

I am able to access your child's records on the GA Registry of Immunization Trasactions Services Website. You do not have to provide a copy unless you would like to. If you are unsure if your child is up to date; please email, call me, or talk to your provider.

If your student's immunization record is not updated prior to our Open House, they will NOT be able to obtain a Class Schedule.

If your student's immunization record is not updated by the first day of school, they will NOT be able to start school. 

When a vaccine is administered, please have the doctor review the updated record form (GA3231). If any errors are found by Forsyth County Schools the record WILL NOT BE ACCEPTED.
