ATHLETIC GUIDELINES:
Physicals
All students trying out for any sports at the middle or high school levels must have a current physical on file with that school. The following forms need to be filled out by a doctor and uploaded to DragonFly:

DragonFly Directions
- Download the DragonFly MAX app from the App Store or Google Play. You can also create an account from your personal computer by going to this link: Dragonfly Link
- Tap “Get Started” and “Sign Up for Free” then follow the prompts to create your parent account with your own email address. Please do not create an account with your child’s name or contact information. There will be an opportunity to add your child at another point.
- Verify your account with the verification ID sent to your email address.
- Tap “Connect to your school” to select “Parent” as your role and search for your child’s school.
- After selecting your child’s school, tap “Join” to request access. A school administrator will approve your request.
- Tap “Set up your children” and follow the prompts to add your kid(s) and fill out or upload the eight mandatory forms.-
**PLEASE NOTE: when you choose a sport for your child, please choose “JUNIOR HIGH” (this is the same as Middle School)
**PLEASE NOTE: when you choose your child’s graduation year, it is their graduation year from HIGH SCHOOL.The following forms need to be completed electronically:
- Medical and Demographic Information (first time only)
- FCSS Athletic Participation Form (beginning of school year)
- Blanket Permission to Participate in Field Trips/Transportation Waiver (beginning of school year)
- GHSA Student/Parent Sudden Cardiac Arrest Awareness Form (beginning of school year)
- GHSA Student/Parent Concussion Awareness Form (beginning of school year)
- GHSA Heat and Humidity Policy for ALL Sports (beginning of school year)
- GHSA Health History Form (beginning of school year)
The following forms need to be uploaded:
- Insurance Card
- Physical Examination (found at top of this page)
- Medical Eligibility Form (found at top of this page)
*Note: If you have an iPhone, open up your Notes app, start a new Note, click the camera at the bottom, click scan documents, take a picture of both forms, click save, and then upload the documents.
The Athletic Director will approve your forms, so please check back frequently. If a form is “Rejected”, there will be a comment added to let you know what needs to be corrected.
Student Insurance
Students are required to have insurance. If you need insurance, you can purchase it through K&K Insurance Group. Please see the information below to get started.

GHSA Constitution and By-Laws
1.40 - LIMITS OF PARTICIPATION1.41 Students must have a certificate of an annual physical examination on file at the school prior to participating in any athletic try-outs, practices or games that indicate the students are physically approved for participation.(a) Physical examinations will be good for twelve (12) months from the date of the exam. EXCEPTION: Any physical examination taken on or after April 1 in the preceding year will be accepted for the following GHSA school year.(b) The physical exam must be conducted by a licensed medical physician, doctor of Osteopathic medicine, nurse practitioner or a physician’s assistant.(c) The exam must be signed by an M.D. or D.O., but the doctor's stamp is acceptable if it is in script, and if information appears elsewhere on the form identifying him/her as a medical doctor.(d) The GHSA requires that member schools use the latest edition of the preparticipation physical evaluation form approved by the American Academy of Pediatrics, et. al., found in the “GHSA Forms Notebook.”Academic Requirements:
Academic Requirements include passing five classes in a six or seven course semester. For Connections classes, the two classes taken in a nine week period will be averaged together to come up with one grade; therefore, the four classes taken over the two nine weeks of the semester will yield two semester grades. A person can not fail more than two classes per semester (7 periods).
Ticket Information:
Tickets for home athletic events can be purchased on the GoFan website at this link, https://gofan.co/app/school/GA13653. Tickets are $5 for adults and students.
** When purchasing tickets on GoFan, please be sure to confirm that the tickets are being purchased for the CORRECT LOCATION. Each game will have two locations, one at Piney Grove as well as one at the opponent's location. Make sure you purchase tickets for the location and game you are wanting to attend**
