Student Proof of Age
A birth certificate is the most common documentation provided to show proof of age for the student. Birth certificates for children born in Georgia can be requested from the Georgia Department of Public Health: Vital Records at 404-679-4702. Birth certificates for children born in other states can be obtained by contacting the Vital Records department within the state of birth.Acceptable documentation for Student Proof of Age are:- Original or certified copy of birth certificate
- Hospital issued birth certificate
- Hospital issued birth record
- Valid driver's license for student
- Passport
- Adoption Record
- Religious record signed by authorized religious official
- Official high school transcript
NOTE: All documentation must display the child's name and date of birth.
