• Certificate of Vision, Hearing, Dental, and Nutrition (Form 3300)

     
    NOTE: As of July 2015, there is a NEW Georgia 3300 form which now includes a Nutrition section. All students from a Georgia school must provide the 3300 form on file from their Georgia school or new GA Form 3300 during the registration appointment.
     
    For enrolling in the 20/21 school year (starting August, 2020) 
    A current and complete Georgia Department of Human Resources Form 3300 (hearing, vision, dental, and nutrition exam) is required by state law for all grades. For student that will be starting Special Needs PK or Kindergarten, the form must be dated August 6, 2019 or more recent.
     
    For enrolling in the 21/22 school year (starting August, 2021) 
    A current and complete Georgia Department of Human Resources Form 3300 (hearing, vision, dental, and nutrition exam) is required by state law for all grades. For student that will be starting Special Needs PK or Kindergarten, the form must be dated August 5, 2020 or more recent.
     
    This form is available from the Forsyth County Health Department located at 428 Canton Hwy, Cumming, GA 30040 (770-781-6900) or a Georgia physician.  The health department and your Georgia physician's office may do all 4 sections of the screening, however some physicians will not complete the dental portion.
     
    Please note, many times during a registration appointment we see incomplete forms with items not filled out by a healthcare provider or dentist. Use the "Detailed Requirements for Form 3300" link below to view a full breakdown of what to look for on your forms. If you see items missing, notify the appropriate healthcare provider to obtain a corrected form.  Children WILL NOT be enrolled unless an accurate and complete form is submitted. This can cause delays to your child's start date and also cause additional trips to the registration center.
