Medications
All medication (prescription or non-prescription) must be brought to the school by a parent or guardian
No student is allowed to have any medication in their possession while at school. This includes over the counter medications (itch creams, cough/cold medication, Tylenol, etc.) For emergency situations, some students may carry their emergency medication with the appropriate required documentation from a physician. Medications must be in their original container/packaging. Medications that are brought in a ziploc bag or other unmarked containers will not be given. Prescription medications must have the pharmacy label attached and the information must match what is written on the Medication Form filled out by the physician. We request that you ask the pharmacist to give you two labeled prescription bottles so that you have one bottle at home and one to be kept at school.
Do not bring medication to school that needs to be given once daily or two/three times a day unless the physician specifically states that the medication must be given at a time that falls during the school day. If medication can be given at home, please do so.
It will be the responsibility of the parent/guardian to inform the school of any changes in pertinent data.
New medications will not be given unless a new form is completed. A new Request for Administration of Medication form must be completed by a parent/guardian. The bottom portion of the form is to be completed by the physician for ANY medication given for more than two weeks, and all prescription medications.
There is zero tolerance for drugs and alcohol in Forsyth County School.
