My Goal Plan:

Using the My Goal Plan template the teacher will assist the student in identifying, monitoring and reaching a goal. As the student and teacher regularly discuss the My Goal Plan and use it throughout the school settings and various environments, the student will begin to self-reflect and monitor his/her progress while gaining skills in setting goals and self-advocacy.
