Important Graduation Forms:

Scholarship Reporting Form - This form is used to recognize scholarships awarded at Senior Awards Night. Please have this form filled out by 5/2/25.

Senior Exit Survey: All students must fill out the Senior Exit Survey by April 22nd in order to get their cap/gown on April 23rd. We will not pass out gowns unless you have completed this form.

Pathway Completion Medallion Applications

Fine Arts Pathway Completion Forms - In order for students to wear a Fine Arts Medallion at graduation, you must turn in both forms to the counseling office by May 9th, 2025

Counselor Recommendation Requests

We encourage all seniors that need a recommendation to complete a Senior Resume which will provide counselors with important information that can be used when writing letters of recommendation. The link is available below. Please check with your college of choice to see if they require letters of recommendation. If your college requires a letter of recommendation, please ask the desired teachers and/or counselor at least two weeks in advance of the application deadline. Teachers and Counselors receive a high volume of requests in the fall, so being proactive is important for a quality and timely recommendation letter. It is also helpful to follow-up with the counselor through e-mail to make sure they are aware of details regarding the letter of recommendation.
