Transcript Course Addition Forms
(Online Course Credit Acceptance)

Students who take Georgia Virtual School or any online tuition-based courses, have an option to have the course grade and credit posted to their transcript; however, if the course(s) is to be used to satisfy prerequisite and/or graduation requirements, the course must become part of the student’s official transcript. Once the credit is posted on the high school transcript, it cannot be removed.

Fall 2024 and Spring 2025 (and all previous terms) Transcript Course Addition Process-Currently Closed

Please note, all forms received by May 30th will be processed, but due to the large volume of forms, some courses may not be added to the transript until August 2025.

If you missed the May 30th deadline for adding a Fall 2024 or Spring 2025 (or previous term) course to your transcript, please check this page again in September 2025 for late process instructions.

• Summer 2025 Transcript Course Addition Process for

• Rising 9th and 10th Grade

• (Classes of 2028 and 2029)

Starting Summer 2025, rising 9th and 10th grade students may only add 1.0 credit from FVA or GAVS to their transcript per semester. Visit the LHS Online Learning Page for more information. 

Rising 9th and 10th grade students will complete the FCS Online Course Student-Parent Pay Credit Acceptance Form (transcript course addition form) in Infinite Campus Parent Portal. 

The form will only be available to students enrolled in FVA or GAVS courses.

Form opens in IC Parent Portal: July 17, 2025

Form Deadline (Form closes in IC Parent Portal): August 22, 2025

Summer 2025 Course Addition Process for Rising 11th and 12th Grade (Classes of 2026 and 2027):

Form opens: July 17, 2025

Form closes: August 22, 2025

Step 2: Fill out the online form linked HERE. 

PLEASE NOTE! BOTH steps above must be completed in order for your online final grade to be added to your Lambert transcript!

You do not need to complete these steps if you are taking a Forsyth Virtual class.

**If you are taking a class outside of GAVS or FVA (Keystone, BYU, James Madison, etc.), you must create an account on parchment.com and request the official transcript from that school to come to Lambert High School.
