K-12 School System Records Requests
Records Request for Current Students and Recent Withdrawals
- Please contact the student's current/most recent school Data Clerk to request records.
- Please contact the Special Education Department (770-887-2461, ext. 310159 or cnorthcutt@forsyth.K12.ga.us) to request Special Education Records.
Records Requests for Inactive Students
- A signed (by a school official or parent/legal guardian) records request listing the document(s) you are requesting for students that have withdrawn may be mailed to:

Records will be mailed (USPS) to you within 10 business days of receipt of a signed Records Request form listing the documents you are requesting.
