VACCINATION REQUIREMENTS FOR ALL UPCOMING 7^TH GRADERS

Georgia State Law requires all rising 7^th Graders must have a Meningitis (MCV4) and a Tetanus (Tdap) vaccine on a GA3231 Immunization Record before entering the 7^th Grade year.

When the vaccine is administered, please have the doctor review the updated record (GA3231).  If any errors are found by Forsyth County Schools the record will not be accepted.
