7th Grade Immunization Requirement
Georgia state law requires all rising seventh grade students born on or after January 1, 2002 to have an age-appropriate Meningococcal ACYW (MCV4), and an age-appropriate Tetanus/Diphtheria/Pertussis (TDaP) vaccine documented by a healthcare provider on a Georgia 3231 Form (Certificate of Immunization), prior to entering the seventh grade.
Students are eligible to receive these very important immunizations as soon as they turn 11 years of age.
An updated Georgia 3231 Form (Certificate of Immunization) is required to be on file with the school nurse before the first day of school.
Immunizations and Georgia forms may be secured from your child’s physician, Georgia Highlands Medical Services School-Based Health Center 678-965-0495, or the Forsyth County Health Department 770-781-6900.
If there is a financial hardship or transportation issue prohibiting the ability to secure these services, please contact Nurse Michelle for assistance.
