End of Year Clinic Reminders
I have enjoyed helping to care for the health needs of your children this year! Here are a few reminders as we approach the last days of school:
Medication Pick up – All personal medication and medical supplies kept in the school clinic must be picked up by a parent or guardian by 12:00pm on May 22nd, the last day of school. These items are not stored in the clinic over summer break and will be properly discarded on the last day of school if not picked up. The school nurse must abide by these guidelines as outlined in the Forsyth County School policy.
Parents of RISING 7th GRADERS - Please contact your student's doctor to ensure all IMMUNIZATION REQUIREMENTS have been met for 7th grade enrollment. If you have recently received a personal message from the nurse regarding immunizations, including a letter by mail, text message, or email, your student's record is missing required vaccines and action is required to be taken before the first day of school. Please email updated immunization forms to mwinsness@forsyth.k12.ga.us as soon as possible. There are community immunization events planned for this summer. New Health Forms/Medications for the Upcoming School Year - With each new school year, it is necessary to update your child’s Health Forms in the school clinic. All students with medical conditions (i.e. asthma, allergies, diabetes, seizures, etc.) must submit new health care plans each school year. Please plan on completing these forms along with your student's doctor over the summer, and turn them in to the school nurse, along with any needed medication. Forms and medications can be turned in during Open House, or when school resumes in August. - If you plan to supply the clinic with medication for your student's use next year, (i.e. Tylenol, Advil, etc.) please complete a Request for Administration of Medication form for each medication. If medication is over-the-counter, only a parent/guardian's signature is required. For prescription medication, or any off-label dosing instructions for over-the-counter medications, the form must also be signed by your student's physician. Please return completed forms and medication to the school nurse by *parent/guardian drop off*. *Students are not allowed to carry medication on or off campus per the Forsyth County Schools Drug Policy. - If you would like your student to carry their own prescribed Emergency Medication while at school, (i.e. inhaler, epi-pen, diabetes supplies, etc.) your student's physician must indicate they have been trained and are capable of carrying and self-administering the listed medication and a completed Authorization for Students to Carry Emergency Medication form must be on file in the clinic. This form is updated annually and requires three signatures: physician, student, and parent/guardian.
