Scoliosis Screening 

It is a Georgia State Law that 6th and 8th Graders be screened for scoliosis - by their doctor or at school

If you do not want your child screened at school, Parents/Guardians Must Complete the Refusal Form by Wednesday, September 6th.

• For a hard copy of the form, you can access the Refusal Form in English OR the Formulario de Rehuso en Española
  • Hard copies must be returned to Nurse Kelly in the clinic  (blank copies also available for pick up and drop off)

All students are required to remove their shirts during the screening

Girls should wear a bra or a sports bra during their screening (highly recommended)
