• All AP Exams Require a Valid Photo ID to Test. 

     AP Student ID Form

    For every AP Exam, all students need to present a recent, valid, photo ID or the AP Student ID Form.

    Acceptable ID Examples:

    1. Valid Government Issued Drivers' License
    2. Valid Learners Permit
    3. Government ID
    4. Passport
    5. School ID if available
    6. Completed and notarized AP Student ID Form

    If a student DOES NOT have one of the above listed photo ID 1-5, then they must complete an AP Student ID Form and have it notarized in the main office with Mrs. Hart.

    The completed AP Student ID Form must be verified and notarized by Mrs. Hart in the front office. 

    • Use the link to the AP Student ID form to download the document or pick one up at the main office at AAI.
    • Follow the instructions on the form (except for the school's official information) and attach the appropriate photo type according to the guidelines on page 2.
    • Well before your first exam, you should bring the completed form to
      • Mrs. Hart or Mrs. Smith in the front office to verify your identity and notarize the document.
      • Ms. Rodriguez will be in the counseling suite to complete and notarize the document.
    • You must bring the notarized and completed form to each exam you take at AAI in place of a driver's License, school ID, passport, or Learner's permit.
