    Online Physical Information: 

    All student-athletes must create a profile with DragonFly Max and complete all required documentation before they are allowed to participate in tryouts, practices, or games/competitions.  Paper copies will no longer be accepted. 

    All forms should be completed electronically in DragonFly Max except for the physical examination form and physical clearance form, both must be signed by a physician and uploaded.
